To receive a “certificate of completion”, students must answer all the questions on the final exam, submit an answer sheet to EMI’s Independent Study Office and score 75% on the test. There are two ways to take the final exam.

  1. To administer the final exam in the classroom, the facilitator should:
    • Contact the EMI Independent Study office at 301-447-1200 or go to Opscan Form Request page to request copies of the Blank FEMA OpScan Answer Form 95-9.
    • Download the final exam questions for
    • Make sufficient copies of the final exam for the class.
    • Mail the completed FEMA OpScan Answer Forms (95-9) to:

  2. To administer the final exam online, provide final exam submission form link at .

To successfully deliver this course you should:

    • Have training and experience in adult learning
    • Take and pass the web-based version of this course
    • Have ICS or EOC training and experience
    • Coordinate this training with your local emergency manager
