Registration
To register for the seminar, please complete and return the screen fillable General Application Form (119-25-1). If you are planning to lodge on campus please note that rooms are filled on a first come, first served basis.
Block 3 – Social Security Numbers are no longer used on the application form, you will need an individual Student Identification (SID) number. The process for obtaining an SID is quick and easy and can be found at this link.
Block 9 – the Course Code is "E-274".
Block 16 – if you are the "State attendee designated by the State Dam Safety Officer" please put that information in this block.
Blocks 23 & 24 – Please disregard "Additional Endorsements".
